چکیده

A multilevel contiguous fracture of the subaxial cervical spine represents the severe trauma, and anterior plate fixation with anterior transpedicular screw fixation is a new and reliable fixation pattern for the treat-ment of acute spinal cord injury (ASCI). We present a case of multilevel contiguous fractures in the lower cervical spine without obvious posterior ligamentous complex injury, but with complete quadriplegia due to a falling accident. Computed tomography revealed multilevel contiguous fractures of the subaxial cer-vical spine including a posterior arch of atlas fracture with the sclerites of C5 vertebral body into the spinal canal with a major compression of the left anterior side of the cord. The patient was treated with a late surgery of C5, C6 corpectomies followed by a autologous bone graft with titanium mesh and surgical stabiliza tion of anterior transpedicular screw-plate system (ATPSP) through anterior approach only. At 1 year postoperative, the patient showed slight improvement except for urinary and fecal incontinence. The finger strength improved to grade 1 of 5 bilaterally, and muscular strength of lower extremities was grade 1 of 5 bilaterally. The latest followup showed the patient paralyzed without any further improvement at 2 years post operative. Anterior cervical plate fixation with anterior transpedicular screw fixation provides a new route of how to treat multilevel fractures of the subaxial cervical spine, and it is feasible. Surgical intervention provides rigid fixation and facilitates early rehabilitation, but the functional recovery of traumatic cervical spinal cord injury remains a difficult problem.

Surgical management of multilevel cervical spinal stenosis and spinal cord injury complicated by cervical spine fracture

BACKGROUND There are few reports regarding surgical management of multilevel cervical spinal stenosis with spinal cord injury. Our purpose is to evaluate the safety and feasibility of open-door expansive laminoplasty in combination with transpedicular screw fixation for the treatment of multilevel cervical spinal stenosis and spinal cord injury in the trauma population. METHODS This was a ret...

Posterior Cervical Pedicle Screw-Rod/Plate Instrumentation Combined with Unilateral Open-Door Laminoplasty for the Treatment of Acute Cervical Spinal Cord Compression Injury: Report of Five Cases.

All patients were acute, multi-segment cervical spinal cord compression injury secondary to subaxial cervical spine fracture. These patients underwent emergency posterior cervical pedicle screw-rod/plate instrumentation combined with unilateral open-door laminoplasty. Well-placed pedicle screws and good cervical canal dilation were achieved in all patients, as assessed by radiography. Patients ...

Atlantoaxial Transpedicular Screw Fixation for the Management of Traumatic Upper Cervical Spine Instability

Objective: The morbidity and mortality are high for traumatic upper cervical spine instability with cervico-medullary compression. In a clinical retrospective study, the clinical and radiographic results of occipitocervical (OC) fusion using posterior atlantoaxial transpedicular screw fixation in 12 patients with traumatic upper cervical spine instability was reviewed. Methods: Twelve patients ...

Cervical screw missing secondary to delayed esophageal fistula: case report.

Although anterior surgical approaches to the cervical spine have become popular and safe in recent years, they also have some complications. We present a case of loss of an anterior cervical plate screw by the natural tracts. The patient was a 47- year-old woman who was operated on for cervical spondylotic myelopathy at another institution. Surgical interference included two levels of anterior ...
